If address is not current: Utility Bill, Lease/Rental Agreement, Pay stub, etc. are acceptable documents.
Other acceptable forms of ID include: Passports, Student ID, Employee Name Badge, Medicare ID, & Military ID.

- Parent or Guardian present at account opening with the information from above.

Sole Proprietors/Doing Business As Accounts
- Legal Name & Physical Address

Limited Liability Corporations
- Legal Name, Physical Address, DOB, & SSN for each Signer
- LLC Employer Identification Number (EIN) & Physical Address of Business
- Articles of Organization - showing how the LLC is organized and by whom
- Articles of Incorporation - showing that the LLC is registered with the State
Corporations
- Legal Name, Physical Address, DOB & SSN for each Signer
- Corporation EIN & Physical Address of Business
- Articles of Organization - showing how the Corporation is organized and by whom
- Articles of Incorporation - showing the Corporation is registered with the State
- Copy of current By-laws
General or Limited Partnerships
- Legal Name, Physical Address, DOB & SSN for each Signer
- Partnership EIN & Physical Address of Business
- Articles of Incorporation - showing the Partnership is registered with the State
- Current copy of the Partnership Agreement
Organizations & Non-Profits

- Organization Tax ID Number (TIN) & Physical Address of Organization
- Articles of Incorporation - showing that the Organization is registered with the State, unless an exception applies.
- Current Meeting Minutes - Specifically indicating name of Organization, who is to be added to the account, and who has authorization to open the account.
